What Is Dry Needling?

Dry needling is a technique used by trained physical therapists to treat musculoskeletal pain and dysfunction. It involves inserting thin, sterile needles — the same type used in acupuncture — directly into trigger points in muscle tissue.

The term "dry" refers to the fact that no medication or injection is involved. The needle itself is the treatment.

How is it different from acupuncture?

While both use similar needles, they're based on entirely different principles. Acupuncture is rooted in traditional Chinese medicine and focuses on energy pathways. Dry needling is grounded in Western anatomy and neuroscience, targeting specific muscular and connective tissue structures based on clinical evidence.

What Are Trigger Points?

Trigger points — sometimes called "muscle knots" — are hyperirritable spots within a taut band of muscle fiber. When compressed, they produce local pain and often refer pain to other areas of the body.

For example, a trigger point in the upper trapezius muscle (between your neck and shoulder) commonly refers pain up into the head, causing tension headaches. A trigger point in the gluteal muscles can mimic sciatica, sending pain down the leg.

Trigger points develop from:

  • Acute muscle overload or injury
  • Repetitive strain from work or sports
  • Poor posture sustained over time
  • Stress and muscle guarding
  • Deconditioning

How Dry Needling Works

When a needle is inserted into a trigger point, several things happen:

Mechanical disruption: The needle physically disrupts the contracted muscle fibers, allowing them to relax.

Local twitch response: You may feel a brief involuntary muscle twitch — this is actually a good sign. Research shows that eliciting a local twitch response is associated with better outcomes.

Neurochemical changes: Needling alters the local chemical environment, reducing the concentration of pain-producing substances and normalizing the pH of the tissue.

Central nervous system effects: Dry needling activates pain-modulating pathways in the brain and spinal cord, producing broader pain relief beyond just the treated area.

The result is reduced pain, improved range of motion, and better muscle function — often within just a few sessions.

Conditions Dry Needling Treats Effectively

Dry needling is used to treat a wide range of conditions, including:

Neck and Back Pain Trigger points in the cervical and lumbar muscles are extremely common and respond well to dry needling. Many patients experience significant relief from chronic neck and back pain that hasn't responded to other treatments.

Headaches and Migraines Tension headaches and cervicogenic headaches (originating from the neck) are frequently driven by trigger points in the upper trapezius, suboccipital, and sternocleidomastoid muscles. Dry needling these areas can dramatically reduce headache frequency and intensity.

Shoulder Pain Rotator cuff dysfunction, impingement syndrome, and frozen shoulder often involve significant trigger point activity. Dry needling is a valuable component of shoulder rehabilitation.

Hip and Knee Pain Conditions like IT band syndrome, patellofemoral pain, and hip bursitis frequently involve trigger points in the surrounding musculature that contribute to pain and dysfunction.

Plantar Fasciitis Trigger points in the calf and intrinsic foot muscles play a significant role in plantar fasciitis. Dry needling these areas, combined with targeted stretching and strengthening, produces excellent results.

Sports Injuries Athletes use dry needling to accelerate recovery from muscle strains, reduce post-exercise soreness, and maintain peak performance throughout a competitive season.

What to Expect During Treatment

Your first dry needling session begins with a thorough assessment. Your physical therapist will identify the trigger points contributing to your symptoms through palpation and movement testing.

During treatment:

  • You'll feel the needle being inserted — most patients describe it as minimal discomfort
  • When a trigger point is contacted, you may feel a brief local twitch or a deep aching sensation — this is normal and typically brief
  • Sessions typically last 15–30 minutes as part of a broader physical therapy visit
  • Some patients feel immediate relief; others experience mild soreness for 24–48 hours before improvement

Most patients notice meaningful improvement within 3–6 sessions, though this varies depending on the condition and how long it's been present.

Is Dry Needling Safe?
